About Racheal Hernandez

Expertise in Management

 

Racheal Hernandez, MAS, FACHE, is a healthcare operator and transformation leader who specializes in translating enterprise strategy into executable care delivery models across complex health systems. Her work focuses on aligning operational performance, workforce capacity, and clinical delivery to support scalable growth in value-based and access-driven environments.


Racheal has led and supported initiatives spanning primary care redesign, immediate care expansion, AI-enabled clinical documentation, and ambulatory capital planning at organizations including Rush University System for Health and NorthShore. Her experience includes implementation of workflow optimization technologies such as DAX and Healthfinch, operational support for COVID-19 clinical trials, and grant-funded health equity initiatives designed to expand access to care in historically underserved populations.


With a leadership portfolio that includes nurses, radiologic technologists, advanced practice providers, physicians, and emerging clinical leaders, Racheal brings a systems-level perspective to workforce strategy and performance improvement. She is particularly focused on helping organizations bridge the gap between strategic intent and frontline execution—designing operational models that enable clinical teams to meet quality, access, and financial targets simultaneously.


In addition to her health system experience, Racheal is advancing community-responsive care delivery through the development of a mobile urgent care model in Kenosha County, with a focus on improving Medicare access and supporting payer-aligned population health initiatives. She is a Fellow of the American College of Healthcare Executives (FACHE) and currently serves as Chair of the Diversity, Equity, and Inclusion Committee for the Chicago Health Executives Forum (CHEF).


Based in the greater Chicago and Kenosha region, Racheal partners with organizations seeking to operationalize growth strategies, improve care delivery performance, and prepare for risk-based reimbursement environments.
